Your Initial Visit

Our office is open by appointment only. New patients are welcome to contact the office to ask questions and book appointments. Many frequently asked questions can be found on this website under FAQ.

In an effort to better serve you, below is a checklist of things to be aware of before your first visit.

• Please print and fill out the Health Questionnaire form before your first appointment and bring it to the office on the day of the appointment. Feel free to contact us if you have any questions or concerns. Please click here for the form.
• The original consent form will be given to you to sign at the time of your appointment.
• Please allow about two and a half to three hours for your first appointment, which includes the intake and initial consultation, and about one hour for subsequent visits.
• Wear comfortable and loose-fitting clothes to the appointment.
• Payment in the form of check or cash is expected at the time of treatment. The fee for the initial visit is $140, and $90 for follow-up treatments.

Cancellation Policy:

In fairness to both other patients and the practitioner, the cancellation policy for both new and returning patients, is to give at least 24 hours notice. Please realize that there are patients on a waiting list for treatment.
